Structure) If you have any questions concerning the requirements of this RFP, please
contact the Buyer listed on the front page of this document. The bidder is
responsible to read the RFP and submit all the required responses. Bid proposals that do not adhere to the following format will be deemed non-
responsive. It is strongly encouraged that the formatting requirements be
reviewed prior to submitting bid proposals. Bidders must respond to all
sections of the RFP as listed below. Failure to respond to any requested
information in the checklist below may result in disqualification from the
bidding process. Bidders shall follow the binder tabbing structure
provided below. Bidders must verify that their Proposal is submitted to the appropriate
location on time per the schedule of the RFP, with one signed original, the
appropriate number of additional copies, and the instructed number of
electronic copies (CD's). Bid proposals shall include three sections, four
attachments, an executive summary and a cost proposal as outlined below. Tab Name Bidder Technical Proposal

|DMB 285 | Form DMB 285 has been signed by an individual authorized to |

| |has been submitted. Bidders shall submit a signed Form DMB 285 with|
| |the following statement in the pricing field "See Price |
| |Attachment". |
|Company | The Contractor must provide a brief overview of the company |
|History |history and experience as it relates to this RFP. This forms the |
| |basis for the evaluation of the qualifications and experience of |
| |the firm and qualifications of assigned personnel. (This section |
| |shall not exceed 5 pages) |
|Company | The Bidder must provide three (3) Company References including all|
|References |contact information. This should include all Subcontractor |
| |Information, if any. |
|Organization| The Bidder must provide an Organizational Chart including all |
| |lines of authority. |
| | |
| |The Bidder must include a brief narrative describing their Project |
| |Management Approach. This forms the basis for evaluation of the |
| |Project Management and Organization. |
|Resumes | The Bidder must complete and submit a Resume Template for each Key|
| |Personnel assigned to this project. Resumes for Key Personnel |
| |should not exceed four (4) pages each. |
|Deliverables| The Bidder shall provide a project plan and a schedule of |
| |deliverables as stated in the RFP. |
|Response to | The Bidder must complete and submit Appendix C. This forms the |
|Functional |basis for the evaluation of the functional solution and the |
|Requirements|approach. |
|Response to | The Bidder must complete and submit Appendix D. This forms the |
|Technical |basis for the evaluation of the technical solution and the |
|Requirements|approach. |
|Statement of| The Bidder shall demonstrate an understanding of the project and |
|Work |present its proposed solution and approach as it pertains to |
| |Appendix A. |
|Case Studies| The Bidder shall provide responses to the Case Studies, as |
| |described in Appendix B, in order to showcase unique capabilities |
| |as a means to assessing the Bidder's resolutions to various |
| |document management concerns. |
|EA Solution | The Bidder shall provide requested information as it relates to |
| |the State of Michigan standards. |
|Article 2 |The Bidder must provide Acknowledgment and/or concurrence with each|
| |term and condition listed in Article 2 of the RFP/ITB document, |
| |with any comments or issues clearly identified. |
| |The Bidder must provide a statement that a Certificate of Insurance|
| |will be provided as a condition of award has been included |
| |(referenced in Section 2.133). |
|Article 4 | The Bidder shall respond all items contained in Article 4, |
| |Certifications and Representations, initialing each paragraph |
| |requiring an initialed response, acknowledging each certification &|
| |representation, and providing all required information. |
|Article 5 | The Bidder shall respond all items contained in Article 5, |
| |Required Bidder Information. | Sealed bids must be received and time-stamped in Purchasing Operations on
or before the time and the due date specified on the cover page of this
RFP. Bidders are responsible for submitting their proposals to Purchasing
Operations on time. Proposals received after the specified due date and
time cannot be considered unless all other bids received on time do not
